Clinical Experience

I have 22 years of extensive clinical experience in advanced urodynamics, female urology, neurourology, endourology and laser surgery, advanced urological laparoscopy, uro-oncology, reconstructive urology, and 7 years experience in kidney transplantation. With availability of excellent infrastructure, progressive & supportive professors and team of residents and colleagues in PGIMER I did some ‘firsts’ in the institute - laparoscopic radical nephrectomy, laparoscopic partial nephrectomy, laparoscopic ureteric tailoring and reimplantation, laparoscopic radical cystoprostatectomy, VEIL, open and laparoscopic extended pelvic lymphadenectomy for bladder cancer, laparoscopic sacrocolpopexy, laparoscopic paravaginal cystocele repair and Burch Colposuspension, transvaginal tape procedures, Botox in voiding dysfunction / incontinence, sacral neuromodulation (Interstim), transvaginal pelvic floor reconstructions (paravaginal cystocele, uterosacral ligament fixation, sacrospinous ligament fixation, etc.). We learned and introduced the concept of ERAS (early recovery after surgery) in major pelvic procedures (notably, and most commonly radical cystoprostatectomy, orthotopic neobladder and extended pelvic lymphadenectomy).

In collaboration with colleagues I developed specialties like urogynecology, urodynamics, neurourology and advanced urological laparoscopy in PGIMER, Chandigarh, NMC specialty hospital, Abu Dhabi, Aster Ramesh multi-speciality hospital, Guntur as well as Medanta Hospital, Lucknow.

ADMINISTRATIVE EXPERIENCE

Firstly as faculty in PGI, Chandigarh I was involved in most procurements for the department. With great deliberations we could procure top-of-the-line equipment to facilitate teaching, training and clinical work – namely (but not limited to) BK medical 4D urological ultrasound with laparoscopic intraoperative transducer, MMS urodynamics with radiolucent motorized chair, HD laparoscopy systems (Karl Storz, Stryker, etc), endourological and laparoscopic systems, C arms and most notably, da vinci SI dual console Robotic surgical system with turn-key project (I left for UAE by the time system was physically installed).

I organized several theme-based workshops with national & international faculty on panel and coordinated several DNB urology examinations.

As head of the departments (Abu Dhabi and Guntur) I have been involved in procurements, coordination and rosters as well as JCI accreditation process pertaining to the department.

Urodynamics – started working on it in Aug 2005 in the department of Urology, PGIMER, Chandigarh (Solar Silver Digital, MMS International, the Netherlands); performing over 350 multichannel urodynamic studies annually by 2013. Uroflowmetry, pressure flow analysis, urethral pressure analysis and videourodynamics were routinely performed by surgical residents under my supervision. I have been the key investigator in over 8 clinico-urodynamical research projects since 2006. We have propagated the concept of volume-corrected average flow rates, need of local population based flow-volume nomograms in women (eponymed PGIMER nomograms), pioneered nomograms incorporating age of the individual (various publications) and revisited practically various concepts of urethral pressure profilometry. Discovered novel plateau detrusor pattern in patients with external sphincter spasm. Regularized urodynamic services at NMC specialty hospital, Abu Dhabi from 2013 and Aster Ramesh Hospital, Guntur from 2017.
  2. Laparoscopy – started working on it in Jan 2006 at PGIMER, Chandigarh. Introduced various laparoscopic urological procedures in the department (namely – radical nephrectomy, partial nephrectomy, pyeloplasty, ureteral reimplantation, ureteral tailoring, ureterocalicostomy, radical cystoprostatectomy, pelvic lymph node dissection, uterovesical & vesicovaginal fistula repair, etc.). The department at PGI was performing over 250 laparoscopic procedures annually. I have introduced nuances in techniques for laparoscopic ureterocalicostomy, ureteral tailoring & reimplantation and VVF/uterovesical fistula repair (many of which have been published). Regularized advanced laparoscopic services in urology and urogynecology at NMC specialty Hospital, Abu Dhabi and Aster Ramesh Hospital, Guntur.
  3. Voiding dysfunction & female urology – started working on it in May 2006. Introduced various techniques for stress urinary incontinence (midurethral slings – TVT, TVT-O, Monarc, Sparc, custom-made slings, Burch Colposuspension, Pubovaginal fascial slings for SUI) and pelvic organ prolapse (vaginal sacrospinal ligament fixation, complete pelvic reconstruction – anatomical / mesh, vaginal paravaginal cystocele repair, open / laparoscopic mesh sacro-colpopexy, anterior uterocervicopexy, etc.) to PGIMER, Chandigarh and then to NMC specialty hospital, Abu Dhabi. Introduced Botulinum toxin A and Interstim for overactive bladder and sphincteric dysfunction (neurogenic and non-neurogenic). Regularized these services at NMC Specialty Hospital, Abu Dhabi and Aster Ramesh Hospital, Guntur.
  4. Others – popularized flexible ureterorenoscopy for large renal calculi and antibiotic stewardship in Abu Dhabi and south India.
Propogation of scientific knowledge -
  1. I have been key person in organization of various theme-based workshops in the department of Urology PGIMER, Chandigarh since 2005 (vide supra), including organizing secy. In three – urodynamics (2006), urogynecology (2008), overactive bladder (2009). I continued the trend to NMC specialty hospital where I conducted several CME’s semiannually since 2013.
  2. I have given invited interviews on All India Radio, UAE radio on topics of urinary incontinence, prolapsed and painful bladder for public awareness
  3. I have been invited to deliver lectures on urodynamics, painful bladder syndrome and laparoscopy (vide supra)

CLINICAL EXPERIENCE

I have 22 years of extensive clinical experience in advanced urodynamics, female urology, neurourology, endourology and laser surgery, advanced urological laparoscopy, uro-oncology, reconstructive urology, and 7 years experience in kidney transplantation. With availability of excellent infrastructure, progressive & supportive professors and team of residents and colleagues in PGIMER I did some ‘firsts’ in the institute - laparoscopic radical nephrectomy, laparoscopic partial nephrectomy, laparoscopic ureteric tailoring and reimplantation, laparoscopic radical cystoprostatectomy, VEIL, open and laparoscopic extended pelvic lymphadenectomy for bladder cancer, laparoscopic sacrocolpopexy, laparoscopic paravaginal cystocele repair and Burch Colposuspension, transvaginal tape procedures, Botox in voiding dysfunction / incontinence, sacral neuromodulation (Interstim), transvaginal pelvic floor reconstructions (paravaginal cystocele, uterosacral ligament fixation, sacrospinous ligament fixation, etc.). We learned and introduced the concept of ERAS (early recovery after surgery) in major pelvic procedures (notably, and most commonly radical cystoprostatectomy, orthotopic neobladder and extended pelvic lymphadenectomy).

In collaboration with colleagues I developed specialties like urogynecology, urodynamics, neurourology and advanced urological laparoscopy in PGIMER, Chandigarh, NMC specialty hospital, Abu Dhabi, Aster Ramesh multi-speciality hospital, Guntur as well as Medanta Hospital, Lucknow.

Firstly as faculty in PGI, Chandigarh I was involved in most procurements for the department. With great deliberations we could procure top-of-the-line equipment to facilitate teaching, training and clinical work – namely (but not limited to) BK medical 4D urological ultrasound with laparoscopic intraoperative transducer, MMS urodynamics with radiolucent motorized chair, HD laparoscopy systems (Karl Storz, Stryker, etc), endourological and laparoscopic systems, C arms and most notably, da vinci SI dual console Robotic surgical system with turn-key project (I left for UAE by the time system was physically installed).

I organized several theme-based workshops with national & international faculty on panel and coordinated several DNB urology examinations.

As head of the departments (Abu Dhabi and Guntur) I have been involved in procurements, coordination and rosters as well as JCI accreditation process pertaining to the department.

Key-contributions –

  1. Urodynamics – started working on it in Aug 2005 in the department of Urology, PGIMER, Chandigarh (Solar Silver Digital, MMS International, the Netherlands); performing over 350 multichannel urodynamic studies annually by 2013. Uroflowmetry, pressure flow analysis, urethral pressure analysis and videourodynamics were routinely performed by surgical residents under my supervision. I have been the key investigator in over 8 clinico-urodynamical research projects since 2006. We have propagated the concept of volume-corrected average flow rates, need of local population based flow-volume nomograms in women (eponymed PGIMER nomograms), pioneered nomograms incorporating age of the individual (various publications) and revisited practically various concepts of urethral pressure profilometry. Discovered novel plateau detrusor pattern in patients with external sphincter spasm. Regularized urodynamic services at NMC specialty hospital, Abu Dhabi from 2013 and Aster Ramesh Hospital, Guntur from 2017.
  2. Laparoscopy – started working on it in Jan 2006 at PGIMER, Chandigarh. Introduced various laparoscopic urological procedures in the department (namely – radical nephrectomy, partial nephrectomy, pyeloplasty, ureteral reimplantation, ureteral tailoring, ureterocalicostomy, radical cystoprostatectomy, pelvic lymph node dissection, uterovesical & vesicovaginal fistula repair, etc.). The department at PGI was performing over 250 laparoscopic procedures annually. I have introduced nuances in techniques for laparoscopic ureterocalicostomy, ureteral tailoring & reimplantation and VVF/uterovesical fistula repair (many of which have been published). Regularized advanced laparoscopic services in urology and urogynecology at NMC specialty Hospital, Abu Dhabi and Aster Ramesh Hospital, Guntur.
  3. Voiding dysfunction & female urology – started working on it in May 2006. Introduced various techniques for stress urinary incontinence (midurethral slings – TVT, TVT-O, Monarc, Sparc, custom-made slings, Burch Colposuspension, Pubovaginal fascial slings for SUI) and pelvic organ prolapse (vaginal sacrospinal ligament fixation, complete pelvic reconstruction – anatomical / mesh, vaginal paravaginal cystocele repair, open / laparoscopic mesh sacro-colpopexy, anterior uterocervicopexy, etc.) to PGIMER, Chandigarh and then to NMC specialty hospital, Abu Dhabi. Introduced Botulinum toxin A and Interstim for overactive bladder and sphincteric dysfunction (neurogenic and non-neurogenic). Regularized these services at NMC Specialty Hospital, Abu Dhabi and Aster Ramesh Hospital, Guntur.
  4. Others – popularized flexible ureterorenoscopy for large renal calculi and antibiotic stewardship in Abu Dhabi and south India.

  1. I have been key person in organization of various theme-based workshops in the department of Urology PGIMER, Chandigarh since 2005 (vide supra), including organizing secy. In three – urodynamics (2006), urogynecology (2008), overactive bladder (2009). I continued the trend to NMC specialty hospital where I conducted several CME’s semiannually since 2013.
  2. I have given invited interviews on All India Radio, UAE radio on topics of urinary incontinence, prolapsed and painful bladder for public awareness
